This position is responsible for clinical operations across the continuum of care, including all aspects of the clinic/urgent care experience, as well as all aspects of medical management. This includes the development, implementation and oversight of on-site care management programs, health education, treatment/clinical decision support, as well as non-clinical aspects of patient-centered care planning and management. The Director of Patient Care Management will manage, supervise and have oversight to all nurse clinicians, care managers , health coaches, pharmacists and other ancillary staff.
Provides direction and management for all aspects of direct patient care received in the urgent care and/or clinic setting, as well as care management teams.
Directly supervises and manages a team of medical and behavioral staff for care management
Supervises clinical nursing staff and/or other specialty clinicians (e.g pharmacist, trainer, dietician)
Supervises non-clinical staff (such as volunteers) as well as provides oversight of the integration of these services with the clinical needs of the patient population
Develops and deploys clinical education to ensure on-going training for staff growth , development and certification requirements.
Works with local market leadership, Human Capital , and managers to successfully recruit, hire, on-board and train clinical and non-clinical staff for all clinic operations and care management functions
Collaborates with all functions to ensure end to end clinical processes match the operational goals and outcomes.
Works with local and national teams to develops and deploy clinic operations/ programs associated with new and existing services at market level
Works with the local General Manager, Market Medical Director and Operations Director to manage and measure health care trend , as well as outcomes measures of the clinic and/or medical management programs
In collaboration with the local General Manger and Sr. Medical Director , responsible for the budget associated with the clinical programs. Manages budget including all equipment, resources, direct and in-direct costs, and, capital requests and requirements.
Analyzes daily and monthly reports to assess staffing and other key indicators needed for clinic and care management operations
Works with the national team, as well as the local General Manager and Market Medical Director , to ensure all programs are implemented within clinical guidelines and specifications
Identifies continuous improvement opportunities and measures ; develop quality improvement initiatives through root cause analysis.
Works with the national Senior Leadership and local market leadership to enhance clinical experience and care management program effectiveness.

Qualifications
- Registered Nurse with strong managerial and leadership experience, in multiple settings .
- Demonstrated knowledge of clinic operations and medical management processes
- Minimum of 8 -10 years management/leadership experience within the healthcare /managed care industry.
- 5-10 years of clinic/urgent care operations/ management strongly preferred.
- Demonstrated experience and knowledge of medical and behavioral integrated clinical programs for care management and quality management
- Bachelor's degree or commensurate experience required. Master's degree or equivalent preferred
- Recognized exceptional interpersonal skills and collaborator with nurses, behavioral health specialists, physicians and operational team members .
- Experience working with senior populations and/or with patients with chronic illness and co-morbidities a strong plus.
- Knowledgeable regarding QA, URAC, and/or NCQA accreditation
- Superior verbal and written communication skills
- Ability to travel approximately 10-20%
- Experience with and demonstrated proficient use of Microsoft Office Suite, web based programs, EMRs and clinical documentation systems.
